Braden Scale

Braden Scale for predicting pressure sore risk

A six-category risk score — sensory perception, moisture, activity, mobility, nutrition, friction/shear. A lower total means higher risk of a pressure injury.

How it gets tested

The direction of the scale is the exam point: low score, high risk. Items also ask which categories it contains and which patient in a list scores lowest.
