Subjective, Objective, Assessment, Plan, Implementation, Evaluation
SOAP extended so the note records not only the plan but what was actually done and whether it worked. A further variant, SOAPIER, adds Revision at the end.
What each letter stands for
- 1 Subjective.
- 2 Objective.
- 3 Assessment.
- 4 Plan.
- 5 Implementation — what was carried out.
- 6 Evaluation — the patient’s response.
How it gets tested
The E is the point: a note that stops at the plan has not recorded an outcome.
