What each letter stands for

  1. 1 Problem — the identified nursing problem.
  2. 2 Intervention — what was done about it.
  3. 3 Evaluation — the response.

How it gets tested

Usually appears in a set comparing charting formats, where the answer turns on which one is problem-oriented.
