Data, Action, Response

The note structure used in focus charting: each entry names a focus, then records the data, the action taken and the patient’s response.

What each letter stands for

  1. 1 Data — subjective and objective information about the focus.
  2. 2 Action — the nursing intervention.
  3. 3 Response — how the patient responded.

How it gets tested

Tested against SOAP and PIE as one of the named charting formats; the discriminator is the word "focus".
