alert and oriented to person, place, time (×3) — and situation (×4)
Shorthand for a mental-status check: the patient is awake and knows who they are, where they are, roughly when it is, and — for ×4 — why they are there.
How it gets tested
Tested as what each "×" adds, and as a level-of-consciousness ladder alongside AVPU and the Glasgow Coma Scale.
